Test Security and Testing Irregularities
Anyone involved in the administration of the test or handling of test materials must sign a PARCC Security Agreement.
See page 12 of the Test Coordinators Manual for a list of secure materials. Students are no longer required to write their names on scratch paper or mathematics reference sheets.
Either use the chain-of-custody form included with your materials or this electronic version, Computer-Based Chain-of-Custody Form.
Test administrators must report test security breaches and testing irregularities to the school's test coordinator immediately. Do not report minor technology issues that staff are able to resolve and continue with student testing (e.g., student not initially assigned the correct est accommodations in the online test format). The test coordinator will complete the Testing Irregularity or Security Breach Form and send it to the District Test Coordinator. Reports must be made to the state within five business days of the incident.
A Post-Test Certification Form must be completed by the test coordinator once the materials are sent to the District Test Coordinator and shredded or recycled. The completed form must also be sent to the District Test Coordinator.
